2010 Spring Registration Form

OPEN REGISTRATION FOR Doc's SPRING SEASON IS CLOSED

All players registering after Feb 26, 2010 will be placed on a waiting list,
with the exception of 8^th grade boys.

Programs and fees

Spring 2010 Fees


	Boys in grades K and 1	$ 395	Girls in grade 2 and below	$ 395
	Boys in grade 2	$ 425	Girls in grades 3 and above	$ 455
	Boys in grade 3-8	$ 570


	*Note: These fees do not include the US Lacrosse registration*
	Both full and partial scholarships are available. No player will be denied an opportunity to play due to financial need. For further information, please contact Coach Levine at mat.levine@citylax.org
All players must be registered with US Lacrosse and must enter their membership number below. To renew your US Lacrosse membership or join for the first time, go to the membership page of US Lacrosse.

Instructions

1. Fill out the form. Please double check all phone numbers and email addresses.
     Include the US Lacrosse membership number.
     Be certain to choose the correct program/team from the dropdown menu.
     It will show the fee for that program/team.

2. Click "Submit Form" only once when you are ready to submit the form.
A confirmation page will be generated.

3. Print out the Confirmation form, sign it and mail the form with a check payable to Doc's NYC Lacrosse covering
the registration fees to: Doc's NYC Lacrosse, 65 West 89th Street, New York, NY 10024
Note: A participant’s registration will be recorded only after receipt of Submission/Confirmation and payment
