2008 Spring Registration Form

Online registration helps in many ways, especially in eliminating errors in email addresses, telephone numbers, etc.
We strongly encourage all families to use the registration form below.

All players must be registered with US Lacrosse. This is a requirement of the program and must be confirmed below. If the player is registered, he/she will need to input their membership number. To renew a membership or join for the first time, go to the membership page of US Lacrosse. You will need your membership number to complete your registration.

Spring 2008 Fees


	Boys in grade 2 and below	$ 335
	Boys in grades 3/4	$ 395
	Boys in grades 5/6 and 7/8	$ 495
	Girls in grade 4 and below	$ 335
	Girls in grades 5/6 and 7/8	$ 395

	*Note: These fees do not include the US Lacrosse registration*

Both full and partial scholarships are available.
No player will be denied an opportunity to play due to financial need.

Instructions

1. Fill out the form. Please double check all phone numbers and email addresses.
     Include the US Lacrosse membership number.
     Be certain to choose the correct program/team from the dropdown menu.
     It will show the fee for that program/team.

2. Click "Submit Form" only once when you are ready to submit the form.
A confirmation page will be generated.

3. Print out the Confirmation form, sign it and send it in to Doc's along with your payment
